NOTES
For registration, please fax or mail the documents below along with your registration form to the organizing secretariat:

1. Copy of last year's subscription receipt, if you are a TOA member.
2. A document proving that you are a resident, if you are a resident.
3. Bank receipt proving that you paid the registration fee.
4. Please contact the organizing secretariat for course registration.

Speakers and moderators will not have to pay any registration fee. However, they will pay the accompanying person fee, if any.

If both spouses are specialists, the accompanying person fee will not be charged.

Conditions for Cancellation
For registration, 50% of the fee will be refunded for cancellation requests submitted until Wednesday, March 15, 2017. Fees for any cancellation requests submitted after this date will not be refunded.
